Discharge Planning

Social workers and our team of therapists are actively involved in discharge planning, ensuring that patients’ transitions back to their homes are as smooth as possible.

Discharge planning begins prior to a patient’s arrival at Forest View, and continues throughout his or her stay. We make sure that when discharged, each patient is fully prepared for the level of care he or she will be receiving, and we work with insurance companies and medical equipment suppliers to help patients get the Durable Medical Equipment they will need when they get home.
