Admissions Process

Forest View admits and treats clients requiring the full gamut of short and long-term, sub-acute, respiratory and rehabilitative services. We have transfer agreements with all major metropolitan area hospitals.

Referrals are made via a New York State “PRI” (Patient Review Instrument) completed by your referring hospital or physician.

We welcome inquiries and visits at anytime, although we recommend calling in advance to assure staff availability for a tour and consultation.
